Medical Records Department
The Medical Records Department processes all patient medical records, as well as, radiology imaging and reports for Atmore Community Hospital, ACH Complete Care, ACH Family Physicians, ACH Primary Care, and Atmore Urgent Care. We have 30 days to complete requests.
Our department is committed to maintaining patient confidentiality and security in accordance with all federal and state regulations. Patient privacy is our top priority. We ensure all records are scanned, verified for completeness, and securely stored within our Electronic Health Record (EHR) system.
How to Request Your Records
We require a written request to process any record release. Requests can be submitted via walk-in, fax, e-mail, or mail.
• Walk-In Hours (Patient Requests only) Patient requests must be done in person with a photo ID: Monday – Friday, 8:00 AM – 12:00 PM (Noon)
• Physician/Other Requests: Monday – Friday, 8:00 AM – 4:00 PM

Note: The department is closed on major holidays and weekends.
Record Release Fees
• Paper Records: $1.00 per page (first 25 pages); $0.50 per page (pages 26+).
• Digital (Disc or Email) per entity: $6.50 flat fee.
Online Access: Patient Portal
Patients who provided an email address during registration can access their records via the Patient Portal. If you gave an email address at any time during registrations you can also self-sign up. The portal includes records from:

• ACH Complete Care
• ACH Family Physicians
• ACH Primary Care
• Atmore Urgent Care
• DW McMillian Hospital
Troubleshooting & Setup:
• New Users: If you did not sign up during your visit, please do so at any registration window or you can be set-up in-person in the Medical Records department with a photo ID.
